The LID number can be found on the establishment’s liquor license. By providing this important number, your establishment will receive credit for your participation! If you do NOT know the LID # please leave the box blank. You will still get credit for your training, just make sure you print out the certificate at the conclusion of the training to show your manager or supervisor!
